Our office participates with many insurance plans. We require co-pays/co-insurance, deductibles and non-covered services to be paid in full on the day of surgery.
We encourage you as the policy holder to become familiar with your policy and benefits. If your insurance requires a referral, please obtain it and bring it with you for your appointment to assure you receive proper reimbursement.
We accept Visa, MasterCard, Discover, personal checks and cash.
Please bring your medical and dental insurance cards and/or completed claim forms with you.
